Job Summary
Has overall responsibility and accountability for patient care oversight and the management of staffing, quality of care and regulatory requirements. Is responsible for Quality Improvement initiatives and spiritual and bereavement programs. Direct reports include as follows: Hospice RNs, MSWs, Spiritual/Bereavement, and Volunteer Programs.
Essential Functions of the Job
- Patient oversight and management
- Conducts an annual review of Hospice program by conducting joint visits with Hospice staff
- Directly supervises Hospice staff
- Develops a strategic plan / begin with a Strengths/Weaknesses/ Opportunities/Threats (SWOT) analysis
- Creates Hospice Budget; writes monthly variance reports in conjunction with Director of Nursing
- Audits all NOEs, Authorizations, Certifications, and Orders
- Verifies correct frequencies and schedules staff to meet appropriate frequencies
- Monitors and changes pharmaceutical formularies as needed via the pharmaceutical provider
- Orientation and Ongoing Education
- Provides education and competency assessment to all Hospice staff
- Daily oversees Hospice documentation for accuracy. Coaches staff so that documentation can be accurate
- Directs timely communication to staff in all areas of the Hospice programs
- Facilitates IDG 100% of the time for:
- Attendance which must include Hospice RNs, LPNs and CNAs
- Accurate record of notes
- Provides oversight to Hospice quality outcomes and overall patient satisfaction of the program and Star Rating
- Monthly review of clinician Dash Boards
- Performs GAP analysis and SHP for Hospice with quality department
- Hospice Compliance / State & Federal
- Leads the Department of Health survey for Hospice. Assists with Department of Health Survey of Home Care and Private Duty
- Participates in committees as assigned
- Participates in development initiative and community outreach
- Creates and maintains On-call schedule
- Annually evaluates Hospice program for its successes and areas of improvement
- Creates plans for development and improvement
- Covers for Director of Home Health and Administrator in their absence
- Administrative
- Reviews and updates Hospice Policies and Procedures
- Quarterly, reviews 15 patient records
- IP Investigation
- Coordinates Hospice Travel Contracts
- HIS Review and Completion
- Assures Clinical Competency of Staff
- Qualifications
- BSN degree
- Two years of experience in home health and/or hospice nursing
- Management experience
- Possesses current license to practice as a registered professional nurse in the state of Colorado.
- Current BLS verification.
- Adept at medical record documentation and Microsoft programs
Employment is contingent upon successfully passing a fingerprint-based criminal history background check through the Colorado Bureau of Investigation and CAPS check
